After submitting the Evaluation Registration Form, our team will review your request and confirm availability. Please note that submitting the form does not guarantee an evaluation appointment. All evaluations must be reviewed and approved by our center.
If the selected date and time are not available, we will contact you to discuss alternative options.
A $50 evaluation fee is required at the time of registration. This fee covers the use of training equipment, facility time, and staff preparation necessary to conduct a proper and safe evaluation.
If your requested evaluation time cannot be approved after review, the evaluation fee will be fully refunded.
I understand, an evaluation is only considered confirmed once it has been reviewed and approved by our team. Payment of the evaluation fee alone does not guarantee an appointment.
